All incoming domestic students must submit health forms and provide proof of immunity against measles, mumps, and rubella. This information is required by law for students to attend WOU and failure to provide this information will result in a registration hold. The required forms may be completed by visiting our immunization page on this website. [If you are unable to access the online forms, you may also print the health history forms from the page (PDF) and submit it by mail or fax.]

The Student Health and Counseling Center is available to all students enrolled at WOU. Students who are registered for six credits or more pay the student health fee. This fee entitles them to routine office visits to the health center and to unlimited counseling sessions at the counseling center.
Students who are enrolled for fewer than six credits can elect to pay the Student Health fee, making them eligible for use of the services at the Student Health and Counseling Center.
Additional fees will be assessed for the following:
*Laboratory testing (blood work, etc.)*Medications
*Medical supplies
*Sports, work or travel exams
*Assessments: vocational, drugs, alcohol, etc.
*Women's health care annual exams

Western Oregon University offers a comprehensive medical insurance plan, available to all students enrolled in six or more credit hours (not including online credit hours) and their dependents.
