In order to receive services from the Office of Disability Services, you need to fill out a request form. The type of form that you use depends upon the situation for which you are requesting services.
If you have been approved for interpreters, note takers, books on CD, or alternative testing, and you wish to receive these services in a classroom setting, you need to fill out a Service Request Form, which is printed on green paper and is often referred to as "The Green Form" (example below). You should submit a service request form as soon as you register for classes to enable us to get services set up in a timely manner.
The Green Form
To fill out the form, please provide your name, address, phone number, e-mail address, Social Security Number, and other requested information on the top part of the form. Next, fill in your schedule of classes (including location and CRN number). Beside each class, place an X in the column of the service (or services) you wish to receive. If you are unsure whether you need to have a service for a particular class, go ahead and request the service. It is easier for us to cancel a service if you do not need it, rather than try to set up a service in the middle of a term. Please complete all parts of the form.
