Travel Authorization Signature Request

Please complete the form below to get an updated travel authroization signature on your I-20 or DS-2019. If your dependents will travel with you, make sure that they have updated signatures as well before traveling. Submit a separate form for each document that needs a new signature.

Signatures must be updated every six months and students should always make sure before they leave the U.S. that the date of the travel signature will still be valid as of the date they plan to re-enter the U.S. from traveling abroad.

After submitting this form, bring your original I-20 or DS-2019 document and your passport to the ISSA office in Maaske Hall. You must submit this form before you will recieve a travel authorization signature or a visa application letter from a Designated School Official. Thank you.
