Change of Major/Minor/Advisor Form

Effective 3/17/16: The Academic Advising and Learning Center will no longer carry change of major, minor, or advisor forms.

You will now need to print off the new change form from the Registrar's website (link below). The new form can be used for either major, minor, or advisor change(s). To change a major, students will need to download and complete the form, then meet with an advisor from the department they are changing to, and obtain that advisor's signature and then submitting it to the department. If you have any questions regarding the new change form, please contact the Registrar's Office at 503-838-8327.
