The following forms are in .PDF format. If you do not have a PDF reader you can download Adobe Acrobat Reader for free from here. Please fill out the forms online then print them and return the completed documents to Infirmary Room 343.
Completion Portfolio Coversheet
Students who enrolled in Honors in Summer B 2012 or later must complete the new completion requirements. This is the coversheet for the required portfolio.
Certificate of Completion (an option for students who enrolled in Honors prior to Summer B 2012)
When students have successfully completed four Honors courses, they may apply for a certificate of completion from the Honors Program. When the certificates are ready students will be sent a confirmation email. The certificates can be picked up in the Honors Office (Infirmary Room 343).
Honors Thesis Submission
Students may submit an Honors thesis for High and Highest Honors designation with your degree.
Research Stipend
National Merit and Achievement Scholars receive a research stipend (up to $1000) which can be used for the purchase of research equipment, or travel expense to conduct your research. Please review the second page of the form for complete details. Scholars who began their studies at UF prior to 2006 may use this money to buy a computer but they must purchase the computer first and then bring in a receipt to the Honors Office to be reimbursed.
Summer out-of-state tuition fee waiver
Out-of-State National Merit and Achievement Scholars can waive up to 9 summer credit fees during their academic career. Forms must be submitted by the last day of drop/add. No retroactive forms will be accepted. **No waiver will be processed for OFFBOOK courses**
