Pre-Matriculation/Immunization Requirements
All new and transfer students are required to submit the Online Medical Clearance Form with documentation of the following prior to the completion of registration.
Required Vaccines

Two doses of Measles, Mumps, and Rubella vaccine (MMR) after your first birthday. (Not required if you were born before 1957.)International Students:

Students with a prior positive PPD must bring a copy of a chest x-ray report, (translated in English) within the last year. Students who have a prior positive PPD or who are receiving treatment for tuberculosis should bring copies of all treatment records, including medical notes, laboratory reports, and x-rays. The cost of the tuberculin screening is nominal.
Recommended Vaccines
Tetanus/Diphtheria/Pertussis (recommended)
Booster: Tdap (preferred) (tetanus toxoid, diphtheria, and acellular pertussis) is recommended with at least five years since the last dose of Td. Administer with meningococcal conjugate (MCV4) vaccine simultaneously if both are indicated.
Booster: Td (tetanus toxoid and diphtheria) is recommended if Tdap is not available or was administered previously.

Meningococcal vaccine (highly recommended)
Meningococcal Tetravalent conjugate; administer simultaneously with Tdap if possible.Any entering student age 25 or under should carefully review the information on the meningococcal vaccine on our website and discuss potential benefits of vaccination with his/her individual healthcare provider.

Quadrivalent Human Papillomavirus Vaccine (HPV) (strongly recommended)
A series of 3 vaccines is recommended for all female college students 11 to 26 years old.

Varicella (recommended)
Either a history of chicken pox, a positive Varicella antibody, or two doses of vaccine given at least one month apart if immunized at the age of 13 or older.
Hepatitis B (highly recommended)
A series of 3 vaccines is recommended. May also be combined with Hepatitis A.

Hepatitis A (recommended)
A series of 2 vaccines is recommended for routine use in all adolescents through the age of 18 and in particular for adolescent and adult high-risk groups (i.e., persons traveling to countries where hepatitis A is moderately or highly endemic, men who have sex with men, users of injectable and noninjectable drugs, persons who have clotting-factor disorders, persons working with nonhuman primates, and persons with chronic liver disease). May also be combined with Hepatitis B in a series of 3 vaccines.
