Complaints

Patients, students, clients, and/or their legal representatives may file a Privacy Complaint at any time, if they feel their privacy rights have been violated.

Privacy Complaint Form
File a Privacy Complaint by printing, completing the form, and signing. Then mail, fax, or scan/email the form to the Privacy Office.
