Requests for Special Medical Accommodations
Housing and Residential Communities is able to consider requests for special accommodation in housing based on medical need, as long as they are submitted by the deadline and accompanied by documentation from the student's physician. Students will still need to complete their housing application through the MyHousing portal. Only one medical accommodation request form is necessary per academic year.
- For housing during the spring semester: December 1;
- For housing during the fall semester:
- February 1 for returning students;
- March 1 for incoming freshmen.
Once completed, the 2016-2017 Medical Accommodation Request Form should be submitted to the attention of Kimberly Sterritt, Associate Director of Housing Administration:

Requirements of the Medical Documentation
To consider a student's request for an accommodation in the student's housing assignment because of a disability, Housing and Residential Communities requires documentation of the student's current medical condition from the treating and licensed clinical professional or health care provider thoroughly familiar with this student's condition and functional limitations and/or restrictions. Said professional or provider shall not be a family member through blood, marriage or other legal arrangement. The form must be completed in full and signed. This documentation will ONLY be utilized by HRC for housing specific accommodations. Additional documentation will be needed by the Office of Disability Services to determine academic accommodations that may be necessary.
UA recognizes that despites its good faith and reasonable efforts to provide reasonable accommodations to its policies, practices and procedures for students with disabilities, students may ultimately disagree with the determination made by HRC. In the event a student with a disability disagrees with the determination made by HRC, the student may request a review by UA's ADA Coordinator. Written appeals of the decision by the ADA coordinator will be reviewed by the Vice President of Student Affairs. The decision by the VPSA is the final institutional review.
